Fern Medicine PLLC (“The Fern,” “we,” “us”) operates fernmedicine.com. This policy explains what information we collect through this website, how we use it, and the choices you have.
This policy covers the website only. Medical information you share as a patient is kept confidential as part of your medical record, as required by law, and is not covered by this policy.
Information we collect
Information you give us. When you fill out a contact or callback form, we collect your name, email address, phone number and your message. Please don’t include medical details in website forms. We’ll collect that information directly once you’re a patient.
Scheduling. Appointments are currently booked by phone or email. Information you share with us to book is used only to schedule and provide your care.
Information collected automatically. When you visit, our servers record basic technical information such as your browser type, device, the pages you view and the referring site. We also use Google Analytics to understand how visitors use the site. Google Analytics uses cookies and collects information such as pages viewed, time on site and your general location. It does not receive what you type into our forms.

We keep contact form messages only as long as needed to respond and for reasonable record-keeping. Medical records are kept as required by New York State law.
